The annual deductible for the Smartsense Plus Standard Rx 6000 plan is $6,000. There is a $9,500 out-of-pocket limit, and no lifetime maximum. For office visits, Anthem waives the deductible for the first 3 visits per calendar year, at a $30 copayment. After 3 visits, and once the deductible has been reached, members pay a 30% coinsurance. Preventive services are covered 100% by Anthem.

The Anthem CA ClearProtection Plus 3300 plan features two deductibles—-one for Inpatient/Surgical services, and one for Outpatient/Professional and Diagnostic Services. The Inpatient/Surgical Deductible is $3,300, and the Outpatient Deductible is $6,800. The Outpatient deductible is equivalent to the out-of- pocket maximum. A combination of services from both the Inpatient and Outpatient deductibles can be used to meet the out-of-pocket maximum.  For doctor’s office visits, two visits are allowed per year with the deductible waived at a $40 copay. After the deductible is met, no coinsurance is required for the remainder of the year. Prescription drug coverage is a tier-based system that includes both generic and brand name drugs.
